Medications at Day Camp

We cannot accept any medications at camp that are not accompanied by the appropriate form completed by a doctor.

  1. Severe Allergy Health Care Plan - If your child has a severe allergy and carries an epi-pen, we need this form completed by a doctor.
  2. Respiratory Health Care Plan - If your child has asthma and carries an inhaler, we need this form completed by a doctor
  3. Other Medication - If your child needs to take other medications during the day (including prescription drugs, OTC drugs, and vitamins), this form must be completed by a doctor. 
AlertAll forms must be uploaded to your Active Parent Portal account. If you have a last-minute registration, forms may also be sent to info@avid4.com or brought to the first day of camp.
